Examinations are available in-person (home or office visits) or via telemedicine.
• Initial Exam (60 minutes) $400.00
• Follow-up Exam (15 minutes) $100.00
• Follow-up Exam (30 minutes) $200.00
• Home Visit $100.00 (additional)
Dr. Bialek does not accept insurance.
Our office will provide a detailed statement for you to submit to your insurance company.
